Intensive Case Manager

The Center for Basic Needs Intensive Case Manager at JF&CS is responsible for supporting low-income individuals and families in obtaining resources to meet their basic needs and in making progress toward economic stability. The resources of Basic Needs include food, legal services and public benefits, emergency assistance grants, fuel and utility assistance and domestic abuse services. The case manager is also responsible for connecting clients to resources provided by a network of Jewish social service agencies. Clients will include elderly, families with children, and people with physical and psychiatric disabilities. Services are strengths-based and flexible to meet the needs of each individual client situation.

Requirements/Qualifications

• Bachelor's degree is required. Master's degree in social work is preferred.
• Two years of full-time or equivalent part-time, professional experience in case management i.e. working with public benefit systems, coordinating services, working with low-income populations.
• Cultural competency and ability to establish effective professional relationships with diverse groups of clients.
• Strong verbal and written communication skills.
• Strong organizational skills and high level of attention to detail.
• Knowledge of Microsoft Office Suite (excel, word, outlook, power point)
• Must have a car

Essential Job Functions:

• Establish a trusting and supportive relationship with the client.
• Work with the client to prioritize needs and create an action plan.
• Connect the client with any and all JF&CS services that meet their needs, including all Basic Needs programs, services for people with disabilities and mental illness, senior services, and supports for new parents.
• Work with the CJP Interagency Liaison to access and coordinate needed services from CJP Partner Agencies.
• Connect the client to community resources including the Department of Transitional Assistance, housing resources, fuel and utility assistance, and other sources of cash and cash-value support...
• Contact service providers on the client's behalf to expedite access to needed resources.
• Accompany the client to appointments as needed to access resources.
• Provide hands-on assistance completing applications and gathering required documentation as needed.
• Review the household budget to identify opportunities to reduce ongoing expenses, introduce payment plans, and avoid incurring penalties associated with late payment of bills.
• Make home visits or meet with client in the community as needed.
• Follows all JF&CS client record and documentation protocols
• Utilizes the JF&CS client data system, ETO (Efforts to Outcomes)
• Contribute to reporting to funders.
• Other duties as assigned
