Housing Case Manager for Housing Access Collaborative Program (HAC)

The Housing Case Manger of Family Aid Boston will work together with an employment specialist and clinical social workers and case managers to rapidly re-house families, and support them in the process of maintaining their housing and becoming economically self-sufficient. As a team member they will also do outreach to landlords, management companies, housing authorities and other housing agencies to strengthen and build collaborations and identify potential housing opportunities for HAC families.

Duties & Responsibilities: 
• Conduct initial assessments of participant families for the purpose of determining housing needs, housing search capabilities, as well as potential barriers to future housing. 
• Work as part of a team to develop an individual housing and stabilization plan (IHSP) that addresses immediate barriers to housing, potential for income generation, and other service areas to best meet family's needs. 
• Provide clinically appropriate interventions to assist family with stabilization and problem solving.
• Determine level and type of financial support needed to house and sustain families while working towards economic self-sufficiency.
• Conduct home visits to support and assist families in implementing their stabilization plan (IHSP). 
• Accompany family to housing interviews and appointments.
• Assist family with all aspects of managing tenancy, including collaboration with landlords. Provide advocacy, consumer education, and crisis management when necessary to prevent housing disruption.
• Help family establish residential stability and strengthen connections to the neighborhood, schools, medical providers, employment services, and other resources as needed. 
• Connect with area landlords, housing authorities, and management companies to develop housing resources for families. 
• Assist staff and clients with landlord-tenant mediation.
• Monitor client progress and complete regular documentation and reporting.
• Obtain proof of income and all other verifications necessary for rent calculations.
• Calculate client rent portion and maintain records of all client rent payments.
• Collect and submit all necessary forms and verifications to accounting department at start-up and on a monthly basis to ensure landlords are paid in a timely manner.
• Provide housing support to the team.
• Implement the program's outreach strategy to include creating alerts, announcements, and information about the program throughout the homeless provider and prevention networks regarding eligibility and openings.
• Actively participate in all staff, team and clinical meetings.
• Participate in supervision and professional development.
• Assume other responsibilities as requested by supervisor.

Requirements/Qualifications:

• BA with 5 or more years housing experience, MA in human services is a plus. 
• Bilingual in English and Spanish, Haitian creole, or Cape Verdean Creole also a plus.
• 2 or more years working with homeless families.
• Ability to work with families/individuals from diverse social, cultural and ethnic backgrounds in a culturally sensitive, team oriented, and strength based approach.
• Demonstrated ability to work in office, field-based, and community based settings with a diverse, homeless population. 
• Excellent organization and time management skills
