Palliative Care Social Worker

We offer an exceptional opportunity for a Palliative Care Social Worker to join the HSL Palliative Care team in Dedham. The Social Worker will participate in providing holistic, interdisciplinary care to patients and families on the Rehabilitative Services and Long Term Care Units. S/he will work with family members to resolve conflicts, develop goals of care, and clarify treatment plans; serve as a liaison between the PC team, Primary Care Social Worker, and other disciplines; and participate in the development and review of appropriate protocols and processes that support the highest quality end of life care.

This 24 hour per week position requires a collaborative, professional, and clinically skilled individual with outstanding interpersonal and communication skills. The position requires LICSW or equivalent, with a minimum of 3 years of inpatient, outpatient, or home based social work experience. Experience in palliative care setting preferred but not required. The schedule must be a minimum of three days, and could be spread over four days if desired.
