Social Worker - Full Time

The Social Worker will provide psychosocial supportive services to residents of the Hebrew Rehabilitation Center at NewBridge on the Charles (NBOC), which include individual therapy and family support. The Social Worker is an integral part of the interdisciplinary team and will identify resident and family priorities, as well as collaborate with the team on the design and implementation of individualized plans of care. The Social Worker will also support staff and assist the Operations Leader in managing challenging situations so as to benefit resident care. The Social Worker will partner with the floor teams to support culture change initiatives. As a member of the management team, the Social Worker is influential in improving processes and outcomes for residents and staff. 

Duties:

Supports the resident and family with admission to long-term care, including facilitating the resident's optimal adjustment to his/her new environment.
Prepares psychosocial assessments. 
Establishes and facilitates support groups.
Completes social service sections of the MDS as required.
Provides individual and family counseling and psychosocial support.
Educates household staff on residents' rights/abuse prevention.
Attends all regularly scheduled staff meetings and committee meetings.
Collaborates with the team to monitor clinical outcomes.
Assists to maintain conditions on the households and neighborhood that foster high levels of staff satisfaction, engagement, and low turnover.

Qualifications:

Masters in Social Work required 
Minimum of 3-5 years experience as a Geriatric Social Worker preferred
Current Massachusetts license as a Licensed Independent Clinical Social Worker
(LICSW ) preferred or Licensed Clinical Social Worker (LCSW)
Excellent written and verbal communication skills
Must be professional, proactive, collaborative, conscientious and results-oriented.
Must have experience in a fast pace work environment, managing acute crises. Must be able to multi-task.
Proficient in the use of pertinent computer software/systems.
