Social Worker

MHPI, Inc. delivers both service-enriched housing and home-based long-term care services to frail elders and those with disabilities. Since its establishment in 1975, the organization was one of the first to provide community-based services to people with disabilities. MHPI enjoys a presence in Greater Boston, Greater Worcester, as well as Southeastern Massachusetts and Cape Cod. EOE

Social Worker to be responsible for case management of Group Adult Foster Care, CBFS clients as well as with a chronically mentally ill population. 
Full Time--some weekend days

Responsibilities: 

• Completes assessments for GAFC clients, updates annually
• Assesses client needs, identifies and links clients to appropriate supportive or medical services.
• Educates clients about what services are available and how to use them
• Provides social work support to clients and their families.
• Makes every other month home visits as part of an individual treatment plan
• Completes documentation on all GAFC clients at least every other month
• Works as a team with staff to promote recovery and rehabilitation
• Assists clients toward completion of IAP goals as appropriate..
• Documents all interactions with clients, including incident reports as needed

Requirements: 

• Bachelor Degree in Social Work or a related field
• 2 years of experience working with elderly and/or disabled in both community and institutional settings is preferred
• LSW--Must have an LSW at a minimum and experience with case management.
• Bilingual English/Spanish preferred.
