Social Worker

The Social Worker, Adult Family Care, is part of a health care team that provides social work and case management for the Adult Family Care (AFC) program. Responsibilities include assessment and evaluation of potential program participants and caregivers, service plan development, care coordination and monitoring, crisis intervention and education of host caregivers and AFC participants.

Requirements:

BSW is minimum requirement, MSW preferred. Possession of, or eligibility for, Social Work licensure required. Must have knowledge of the home care system and long term care issues as well as excellent organizational and communication skills. Experience using Microsoft Word essential. Must be willing and able to use own vehicle. Bilingual a plus (Haitian-Creole speaking preferred).
