Newton Wellesley Hospital Population Health Behavioral Coach

As an integral member Population Health Management Initiative, the Population Health Behavioral Coach assists in the integration of behavioral health in the primary care setting and is supportive of other Population Health Management initiatives.

Consistent with a Patient Centered Medical Home (PCMH) team based care model, the Population Health Behavioral Health Coach collaborates with all members of the Population Health Management Team including providers in the primary care practice and consultative mental health disciplines to provide behavioral health management for adults. The coach will work collaboratively with a multi-disciplinary team to implement evidence-based services for primary care patients utilizing tools and training from the IMPACT Collaborative Care model and Partners HealthCare sources.

This position requires a strong working knowledge of coaching techniques including motivational interviewing and behavioral activation as well as common mental health conditions, psychosocial issues and chronic diseases which impact overall health and well being. Experience should include a minimum of two years experience in a mental health setting.


  1. Bachelors with two years experience in a mental health setting.
  2. Masters degree preferred in Social Work or Mental Health Counseling
  3. Two years previous experience in a mental health setting preferred
  4. Background or familiarity with
    1. Motivational interviewing and coaching
    2. Behavioral activation
    3. Depression and anxiety management
    4. Addiction and substance abuse
    5. Management of patients with complex medical conditions
  5. Experience working with a variety of community agencies and resources preferred.
  6. Ability to work effectively in a complex fast paced medical environment and multiple practice locations.
