Social Worker/Case Manager: Community Based Shelter Program

The Social Worker provides supportive services to families in scattered site and co-shelter programs for families in crisis. Develop and implement plans that assist families' transition to permanent housing within defined time periods. 

Duties & Responsibilities: 
1. Provides assessment of stabilization and housing needs, and develops plan to best meet housing and family stabilization goals. 
2. Conducts home visits to support family, monitor property, and implement plan.
3. Develops understanding of strengths and limitations of families served. 
4. Leads or co-facilitates groups. 
5. Serves as liaison to schools, medical providers and others to assure best level of community integration.
6. Provides family intervention to identify and assist family with problem solving, crisis intervention and other needs. Conducts family meetings as needed. 
7. Provides intensive case management to maximize eventual stability in permanent housing.
8. Develops plans according to contract expectations. 
9. Participates in all staff, team and clinical meetings. Maintains clear, non-judgmental documentation in clients' files.
10. Participates in supervision, and professional development as determined in performance evaluations. 
11. Covers 4 hour weekly coshelter desk coverage shift

Qualifications: MSW Degree preferred, with experience in social services. Bilingual/bicultural a plus. Ability to work in office and field based settings with a diverse, homeless population. Flexibility and compassion essential.
