CLF Care Coordinator

Catholic Charities' Aging Case Management program assists seniors at every point in the spectrum of care whether they are transitioning back home from a hospitalization or are experiencing an age-related decline in health that will require ongoing support. Case management provides connections to community care services like home-delivered meals, environmental safety checks, patient advocacy, and psychological and emotional support. 

The Care Coordinator provides care coordination services for a caseload of 30-40 Community Living Fund (CLF) clients. S/he is responsible for the modified assessment of clients with multiple medical and psychosocial problems. S/he plans for and monitors purchase of goods and services to support community living. S/he works collaboratively with the community case manager.


• Conducts modified assessments of the client, including; psychosocial, health; including mental health, environmental and spiritual needs.
• Writes modified assessments and develops a client -centered care plan that is consistent with the Community Living Fund (CLF) guidelines as outlined in the Policy and Procedure Manual.
• Conducts home visits. 
• Identifies, arranges for, and monitors appropriate purchases of goods and/or services.
• Meets with clients at least monthly
• Documents via progress notes and all care coordination activity regarding identified problems within 24 hours, adding any new problems to the service plan, as needed.
• Maintains required paperwork and follows a clear, concise, and consistent system of charting to allow for continuity of care.
• Evaluation for client Purchase-of -Services needs and follow-up to determine if services have been provided in a timely manner.
• Educates clients and significant others about resources.
• Establishes and maintains open and effective communication with Long term care manager. 
• Provides appropriate information on all significant aspects of individual client care and program operations, while maintaining necessary confidentiality.
• Refer to CLF Intensive Case Management if needs exceed already involved providers.
• In collaboration with the client, caregiver, and involved services, terminates clients when 
appropriate. Documents the process as required. Data entry into CODI Systems, and completion of monthly reports.
• Participates in research studies and data collection, as required.

Education & Experience:

• Bachelor's degree in Social Work, Psychology, Counseling, or related fields 
• One year of experience in providing direct services in a social service organization
• Minimum one year's experience providing client case management services with the aging population and/or service programs.
• Certified in Red-Cross basic first aid, and CPR are required which may be obtained within 30 days of hire. 

Knowledge, Skills & Abilities:

• Experience and understanding of the medical and psychosocial problems of functionally impaired adults and older adults.
• Experience working with individuals with mental and/or behavioral health diagnoses and substance use disorders highly desired.
• Excellent communication and presentation skills relating to functionally impaired adults and older adults, their support system and teams of health professionals
• Demonstrates case management skills and experience in the community health care delivery system.
• Detailed oriented and good problem solving skills and the ability to prioritize multiple tasks.
• Knowledge of community resources.
• Desire to work with and sensitivity to the aging population and diverse cultures.
• Bi-cultural and bi-lingual in Chinese preferred.
• Computer literate (IBM).
• Promote a positive, professional and organizational image in the community.
• Ability to work under pressure.
• Ability to work as a member of a team.
• Valid CA driver's license and personal car preferred.
• Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in cross-cultural situations.
