Medical Case Manager

We offer individual, group, couple, and family therapy; psychiatric consultation; and addiction services. The following position is available in Fenway Health's Behavioral Health Department: Medical Case Manager

The Medical Case Manager provides and coordinates support, counseling, advocacy and community resource services to medical department primary care patients within an innovative, multidisciplinary medical and behavioral health service.

Typical duties include: assisting medical patients and HIV-positive clients with applying for entitlement programs; providing client advocacy; serving as liaison to external programs; referring patients to support services; acting as a member of medical care teams to ensure comprehensive patient care; completing psychosocial assessments and service plans; supporting grant funding requirements; and completing all required administrative tasks, such as documenting interactions with clients and completing relevant forms.


• BSW or BA in psychology or related field
• Prior experience providing case management in a health care environment preferred
• Interest in or experience with LGBT issues and the primary care needs of HIV-positive people
• Demonstrated organizational skills, attention to detail and ability to respond to patient and provider needs in a busy primary care practice
• Experience working in an ethnically, culturally, and racially diverse environment preferred
• Ability to work harmoniously with diverse groups of individuals required

Salary: Commensurate with experience.
