Case Manager & Outreach Coordinator

The Case Manager works as part of a team to provide direct supports, education, advocacy and supervision for young adults both individually and in small group settings, in accordance with the mission of The Haven Project.


Job Type: Part-time

Reports to: Program Director - Emily Urbina
Pay Rate: $16 - $18 per hr.
Proposed Hours (some flexibility available) :
M- 11-5:30pm
T- 10-6:30pm
W- 10-6:30pm
Th- 10-6:30pm

The Haven Project's Mission is to equip and empower homeless unaccompanied young adults 17-24 North of Boston with the skills and support they need to be successful. Intensive case management, vocational training and educational interventions are key components of our strategy to address the needs of these high-risk homeless young adults.

Essential Duties and Responsibilities:

  • The Case Manager will work to keep clients focused and actively completing basic case management goals and objectives in accordance with the 6 main areas of services we provide: Basic Needs, Place to Live, Employment, Education, Health and Community.
  • Provide hands on job search, resume writing, and life skills activities
  • Provide a safe, productive, social environment for program participants.
  • Under direction of the Program Director, work with young adults to develop life skills
  • Develop agreements and individualized service plans with the youth and family when appropriate for participation in the program
  • As directed, contact youth via phone and social media
  • Maintain good communications with collateral services providers and family members.
  • Facilitate group activities, life skills classes, and recreations activities for the youth involved in the program in coordination with the other project staff
  • Ensure the safety of program participants by working with the team to enforce program safety protocols.
  • Participate in activities and efforts to promote the Haven Program, including participation in outreach activities
  • Document program activities as needed for program evaluation
  • Attend staff meetings as required.

  • Manage all aspects of The Haven Project's street outreach efforts including management of outreach staff, stipended youth participants and volunteers.
  • Recruit volunteer groups to help with outreach endeavors in Lynn, Salem and surrounding communities as directed.
  • Use proven outreach strategies to provide direct intervention to street youth where they congregate. Ensure strategies are language appropriate, culturally sensitive and respectful.
  • Respond to immediate needs of youth in crisis through provision of food, clothing and referrals to medical treatment, shelter, and mental health providers.
  • Develop educational and marketing materials to attract and educate homeless young adults about their educational and legal rights and intervention services.
  • Coordinate backpack purchases and donations and service information to meet immediate needs of homeless unaccompanied youth.
  • Coordinate community outreach activities including developing street outreach schedule, daily client contact for identified youth, general flyering, staffing tables at community events, and meeting with shelter, police and community partners,
  • Refer street and homeless youth to The Haven Project Drop in Center for support services, education, and skills will increase self sufficiency.
  • Maintain good communications with collateral service providers.
  • Follow established operational procedures and program protocol for client management, documentation of activities and volunteer management.
  • Participate in weekly staff meetings.
  • Other tasks and projects as assigned.


  • Possess computer competency & Microsoft office competency
  • Minimum Associate's degree or 8-10 yrs. working with at risk populations
  • Spanish speaking preferred
