Council on Aging Outreach Worker/Case Manager

The City of Attleboro Council on Aging seeks an outreach worker/case manager, who working under the direction of the Council on Aging Director, identifies the needs of senior citizens, provides case management and assistance accessing services, advocacy and education to enhance the quality of life for these citizens.

Salary Range: $20.70-$27.82 per hour

Duties include conducting interviews at the office or in the homes with senior citizens to assess their service needs. Provides access to services and case management for identified senior clients. Provides advocacy and education for clients and the elder population. Coordinates services and programs with area agencies serving elders and disabled population. Maintains client records. Promotes and coordinates events, programs, and activities, to address social, medical, and psychological needs of the elder and disabled populations. Assist the Director in supervising volunteers and interns for programming offered by the department. Must be a good listener and sensitive to the emotional and physical needs of elders. Must be available to respond to elder needs during emergency situations, including storm closings. Must maintain confidentiality Must complete a CORI check and be dependable. Performs work as required.
Requires a thorough knowledge of elder issues and resources. Familiarity with federal, state, and local regulations and programs relative to elders. Must have ability to conduct assessments, perform crisis management, and disseminate information to appropriate individuals and agencies.

Requires Bachelor's or Master's degree in social work or related field and one or more years' experience as a social worker or case manager working with the elderly in a social services capacity or any equivalent combination of education and experience. Successful candidate must be a licensed social worker or have the ability to obtain license within an agreed to timeframe, have a valid drivers' license, Shine Counselor Certification, and CPR/AED certification.
