Team Up Behavioral Health Clinician

The Team Up Behavioral Health Clinician will function as a core member of a collaborative care team that involves the patient and his/her family (when appropriate), the patients' primary care provider, nursing, and community health workers. The Team Up Behavioral Health Clinician will work with children, adolescents, and families and will be expected to work in various modalities such as: individual, family therapy, and group work.

The Team Up Behavioral Health Clinician must be willing to work as an integral part of an integrated health team. Patients being referred to the Team Up Behavioral Health Clinician (optimally via warm hand-off) by Brockton Neighborhood Health Center (BNHC) medical providers are at high risk due to medical diagnoses, frequent use of hospital emergency rooms; and/or have behavioral health/mental health, social or environmental concerns that impede their overall health. Knowledge of CBT, Solution-Focused Therapy, trauma-informed care, and experience with or willingness to learn about early childhood mental health and parent/caregiver- childhood care.

The Team Up Behavioral Health Clinician will provide comprehensive consultation/assessment and bio-psychosocial evaluations on an urgent basis, support and linkage services, advocacy, education, brief interventions, counseling, and information in order to ensure adequacy of health care, increase access of services, enhance physical and psychological well-being and facilitate patient self-care.

o Promote wellness in BNHC's pediatric and OB populations o Provide culturally humble assessment, consultation and brief intervention for psychological/psychiatric problems and/or disorders and diagnostic clarification utilizing DSM-5

o Evaluate crisis situations and apply appropriate interventions and treatment planning
(Suicidal/Homicidal Risk Assessment)
o Assist the primary care team in developing care-management processes such as
guidelines, disease management techniques, case-management and patient and family education to improve self-management of chronic diseases

o Provide comprehensive mental health and substance use disorder
assessments/consultations, confirmation of diagnosis, consultation, and brief intervention for less-complex presentations of behavioral conditions

o Consult with primary care provider to provide effective treatment planning and assist
clients in successfully achieving goals

o Provides timely feedback to the primary care provider verbally and through active
documentation of patient progress and diagnostic information in the treatment chart

o Actively participate in meetings to support implementation and improvement of BNHC's integrated care model o Teach patients, families, and staff care, prevention, and treatment enhancement techniques

o Assist in the detection of "at risk" patients and development of plans to prevent further psychological or physical deterioration
o Provide an array of clinical services for general behavioral health problems and areas of behavioral medicine (treatment planning/provision) to assist patients with meeting goals including, but not limited to:
• Behavior and Mood Management
• Management of family dynamics
• Autism Spectrum Disorder
• Panic & Anxiety Management
• Substance Use Assessment & Intervention
• Stress & Anger Management
• Healthy Behavior and Disease Management
• Medication Adherence
• Chronic Pain Management
• Insomnia and Sleep Hygiene
• Management of High Medical Utilization
• Psychosocial & Behavioral Aspects of Chronic Disease
o Facilitate consultation between in BNHC Psychiatrist (in appropriate cases) and PCP and co-management of patients

o Facilitate consultation between community-based social services, community based mental health, substance use, and psychiatric providers and co-management of patients

o Participation in Clinical Treatment Team to assist with treatment planning to provide
comprehensive care for patients and families

o Participation in Administrative Treatment Team meetings to identify problems related to
patient services and make recommendations for improvement

o Help PEDI/OB BHI Team plan and run family engagement activities/ events.
o Assistance with outcome measures and data collection (when requested)

o Documentation of Services to include behavioral health services and documentation in
EHR as appropriate o May provide supervision to other Behavioral Health staff (when appropriate)

o Perform other duties as assigned

Additional Requirements

1. Excellent working knowledge of behavioral medicine and evidence based treatment for medical and mental health conditions (particularly for Pedi/OB populations)

2. Ability to work through brief patient contacts as well as to make quick and accurate
clinical assessment of mental and behavioral conditions.

3. Should be comfortable with pace of primary care, working with interdisciplinary team
and have strong communication skills.

4. Participate in required trainings as needed.

5. Good knowledge of psychopathology and psychopharmacology.

6. Ability to design and implement clinical pathways and protocols for treatment of selected chronic conditions.

QUALIFICATIONS: Must be licensed or currently seeking licensure, in Social Work (LCSW, LICSW), Mental Health Counseling (LMHC), Psychology (PhD, PsyD). Language and cultural diversity (Cape Verdean Creole, Portuguese, Haitian Creole, French, and Spanish) preferred.
