We are seeking a Primary Care Social Worker to support the Population Health Management department in caring for complex patients and play a key role in triaging patient needs identified through screening for social and behavioral determinants of health. This position acts as a resource to assist patients in navigating the healthcare system and works closely with our Population Health Management Teams to handle patient case management and collaborate with community resources.

Essential Functions of the Position (Illustrative Examples Included)

  • Coordination of Patient Care
    • Provides brief screening and assessment in person or telephonically to patients including but not limited to assessing for and responding to social and behavioral determinant of health needs
    • Assesses and coordinates the healthcare, educational, and social needs of the patient/family over time and provides updated care plans specific to patient goals
    • Connects patients to community resources for a range of psychosocial and socioeconomic concerns affecting adherence to recommended treatments
    • Ensures continuity of care through different systems of care, with an emphasis on behavioral health care coordination
    • Maintains an in-depth understanding of medical insurances and benefits
    • Assists patients with transportation needs
    • Assists with MOLST and durable power of attorney forms
    • Documents outreach, goals and interventions within the electronic medical record
    • Monitors case management progress through periodic follow up for appropriate patients
  • Collaborates with the practice-based care team, central, and specialty care teams
    • Collaborates with PCP, patient and members of the healthcare team, including required health care services outside of Coastal Medical
    • Ensures open communication regarding patient status with physicians, office staff and patients' families
    • Participates in care conferences with care teams for complex patients
  • Attends required trainings and Behavioral Health team meetings as scheduled
  • Performs other job-related duties as required

Education and Experience: 

  • BSW required, MSW preferred
  • Prior experience working as a social worker in a medical setting such as a primary care office or hospital-based setting
