Case Manager School Based

Reports to the Program Coordinator and works with the Senior Case Manager to provide intensive case management to vulnerable 8-13 year old students and their families within a school based setting.  Able to implement the tasks identified on the individual change plan for each student and revise and amend as clinically necessary or recommended by supervising clinician.   Identify family needs through clinical assessment and refer to appropriate services and/or assist families in navigating social service programs.  Identify, develop and facilitate engaging and age-appropriate activities for an after school setting which are trauma informed and developmentally applicable.

Essential Duties and Responsibilities:

  1. Outreach and engage children and their families referred by school administration, teachers and law enforcement to participate in the MassSTART program of the Winthrop Public schools.
  2. Establish weekly individual and group contact with each student on case load.
  3. Maintain weekly communication with family, teachers and collaterals involved with client.
  4. Conduct home visits as needed and may require more frequent home visits if crisis arises.
  5. Provide information and referral, resource advocacy, and other direct services to help families promote healthy habits to reduce the risk of addiction and criminal involvement. 
  6. Assist collaterals in supporting families as they strive to meet treatment goals and maintain appropriate communication with respect to confidentiality.  
  7. Participates in family events, after school field trips, and other incentive based activities for clients and families.
  8. Provides education, advocacy, and support on child and parenting issues and information to client families.
  9. Utilizes mediation techniques to help families solve conflicts.
  10. Assists in crisis intervention.
  11. Provide consultation to partnering agency staff (Winthrop schools guidance and adjustment counselors).
  12. Maintains all confidentiality and all other professional standards.
  13. Attends staff meetings and bi-monthly clinical reviews as required.
  14. Prepares and maintains documentation.
  15. Transport (drive) clients throughout the community to attend to doctor appointments, shopping needs, important errands, etc...
  16. Provides on-call (beeper) coverage as required.
  17. Some weekends / evening hours required.
  18. Performs other duties as may be assigned by Senior Case Manager or Program Coordinator.

Position Requirements:

Education and Experience Required:

High School diploma/GED required, Bachelor's Degree in a related field preferred or at least three years of direct training and experience working with young children, adolescents, and families.  Previous experience with crisis intervention, trauma sensitive populations, family preservation work, and mediation desired. Must have a valid Massachusetts Driver's License.

Knowledge:

Knowledge of developmental task associated with latency and adolescents age children.  Awareness of the behavioral implications of trauma in adolescents and within family systems.  Familiarity with local support programs and the State-wide social service system.  appropriate manners to carry out crisis intervention and mediation.

Skills and Abilities:

  • A natural desire and ability to engage and work with a vulnerable adolescent population.
  • Must have a valid Massachusetts Driver's License.
  • Ability to intervene in crisis intervention situations.
  • Maintain confidentiality of all consumer information.
  • The ability to maintain a pleasant and professional attitude with fellow employees.
  • Ability to be organized in scheduling/time management to meet productivity standards.

Work demands:

  •  Off-site travel is necessary. Urban home visiting required.
  • Individual must have normal vision and hearing.
  • Must have full use of both arms and hands.
  • Must be able to walk and climb stairs.
