Social Work Coordinator

Kit Clark Senior Services' Adult Day Health Program is looking for a Social Work Coordinator.

Responsibilities:

  • Coordinate intake, eligibility and admission process for the ADH program, utilizing BayCis and MassHealth Eligibility Verification System and Virtual Gateway.
  • Inform participants of and refer participants to available community services.
  • Participate in the completion of comprehensive assessments and plans of care.
  • Maintain documentation in the clinical record including psychosocial assessment upon admission, quarterly social work note and progress notes related to significant changes and services.
  • Provide, as appropriate, coordination of the medically-related physical, mental, and behavioral service needs of participants at the time of enrollment, during treatment and care in the program or at the time of discharge
  • Assist with any personal, social, family or adjustment problems the participant may experience at the Program.
  • Refer participant or family to the appropriate community resources if the participant or the participant's family requires specialized counseling.
  • Maintain communication with other service provider agencies to ensure continuity and coordination of care.
  • Assist as necessary in the delivery of other adult day health services.
  • Assist in overall operation of program and activities as needed.
  • Participate in in-services and contribute to staff development activities.
  • Daily use of electronic records and billing systems.
  • Other duties as assigned by Site Manager and/or ADH Director.

Qualifications:

  • LCSW or MSW with LCSW pending
  • One or more years' experience working with the elderly or adults in a professional capacity.
  • Computer proficiency required.
  • Bi-lingual skills a plus.
  • Must have physical exam within the last 12 months and every two years, TB screening and flu vaccine annually, CPR certification and Basic First Aid.
  • Ability to work collaboratively as a member of multidisciplinary and cross-functional teams.
  • Ability to function as an effective change agent.
  • Demonstrated ability to work effectively and collaboratively with a culturally diverse population of clients, other care providers, family and community members.
  • Ability to function under pressure in a fast-paced human service environment.
  • Ability to negotiate and resolve differences.
  • Ability to be flexible, open and responsive to ongoing industry changes.
  • Ability to articulate and communicate Bay Cove/ Kit Clark's mission and strategic direction in a consistent and enthusiastic manner to departmental staff.
  • Ability to identify opportunities and obstacles and develop effective, creative solutions to pursue opportunities.
