Bilingual Assessment Care Manager - Adult Family Care Program

Somerville-Cambridge Elder Services promotes the right of all individuals to live with dignity, in the setting of their choice by offering older people, younger people with disabilities, and caregivers the information, services and support needed to make choices which enhance health, well being and independence.

Since 1972, Somerville-Cambridge Elder Services (SCES) has been the area's aging information and service center. SCES helps older people and people living with disabilities remain safe and independent in their own homes by providing a wide range of supportive services, as well as information and advice.

SCES services are available to older residents of Cambridge and Somerville, as well as their caregivers. SCES is a state-designated Aging Services Access Point (ASAP) -- formerly known as a Home Care Corporation (HCC) -- and a federally-designated Area Agency on Aging (AAA).

We are currently hiring for a Bilingual Assessment Care Manager to join the AFC Team.

The Bilingual Assessment Care Manager is part of a health care team that provides case management for the Adult Family Care (AFC) program. Responsibilities include assessment and evaluation of potential program participants and caregivers, translation/interpreting, home visit coverage, completion of special projects, and care coordination.

Qualifications :
A BSW is the minimum requirement. Possession of, or eligibility for, Social Work licensure required. MSW preferred. Knowledge of the home care system and long term care issues required. Excellent organizational and written and oral communication skills required. Ability to work collaboratively as part of a team essential. Knowledge of human development, including the areas of physiology of aging, mental health, and substance abuse essential. Experience using Word and strong data entry and work-processing skills essential. Bilingual in Spanish, Portuguese, or Haitian Creole a plus. Must be willing and able to use own vehicle.

Mid $40s and an excellent benefits package including a 35-hour work week, medical, dental, agency paid life, short/long term disability, 401(k) with agency contribution, earned leave accrued at a rate of 24 days per year and 13 holidays.
