Social Services Director

Vero Health & Rehab, Parkway, a 141-Bed Skilled Nursing Facility, currently has an exciting opportunity for a well-qualified Social Services Director with an MSW and an LICSW from the Commonwealth of Massachusetts to join our amazing healthcare team! The Social Services Director, along with the Social Services Assistant, will work with our Team to ensure that all Medical, Social and Emotional needs of our Residents are met and maintained in accordance with our Policies and Procedures as well as all applicable State and Federal Laws and Regulations. The Social Services Director will maintain documented Progress Notes of all interactions and planning of care in the Resident's chart. Additionally, the Social Services Director will provide Psychosocial Assessments, intervention, and/or treatment and Discharge Planning to Residents and their Families to assist them in coping with their hospitalization, illness, diagnosis and/or life situation and meet the continuing needs post-hospitalization. The Social Services Director will assist Residents and Families to resolve personal, financial and/or environmental difficulties which interfere with the Care Management process as well as provide crisis intervention as needed.

Additional Requirements the Candidate should possess are, but not limited to:

  • Have a strong understanding of referral sources for varying levels of need such as: Durable Medical Equipment Companies, VNA, Elder Services, Psychiatric services/Psychiatric programs and housing;

  • Versed in PASSARS, Healthcare Proxies, Guardianships, Minimum Data Sets, MOLST forms and OBRA Assessments, Resident Rights, Durable Power of Attorney, Health Care Proxy and DNR Directives;

  • Able to meet with Residents and their Families regarding treatment, aftercare planning, and long-term care planning and then make contact with these sources to establish these services;

  • Able to work with a Multidisciplinary Team as well as be able to work autonomously with day-to-day responsibilities;

  • Able to assist with Admission Process and/or Referrals;

  • Strong written and verbal communication skills.

Qualified candidates preferably should have an MSW from an Accredited School of Social Work, an LICSW from the Commonwealth of Massachusetts and at least one year of experience in Long-Term Care. We have onsite parking as well as being located conveniently between two MBTA Bus stops: Route 37 stops to our north at Baker Street and the VFW Pkwy and to our south at Charles Park Road and the VFW Pkwy. Each stop is less than a ½ mile away or about a 10-minute walk through our tree-lined streets.
